Below are descriptions of each of the application types that may be needed when preparing to complete your desired project. The forms are located on the menu to the right. You may select the needed forms and print them.

Building Site Application

A Building Site Application is applied for when:

  • You are developing raw land.
  • Your are expanding your current residence. Note that it is considered an expansion when you need to increase or modify your drain field.
  • You are establishing records

Only a licensed septic designer can submit a Building Site Application on your behalf.

Building Clearance Application

Anytime you wish to do an addition to your property a review must occur to ensure that the addition does not encroach upon any on-site related areas. By filing a Building Clearance it will initiate a review by the Health District related to all on-site and drinking water issues.

Building Site Extension


A Building Site Extension is applied for when a current Building Site Application is about to expire, and is still within the allowable extension period. Original Building Site Applications are viable for a 2 year period, and extendable for 2 one year extensions.

Commercial building clearance.

Commercial Building Clearances are used when a new business occupies a facility or residence. It is utilized to evaluate if the proposal will fit with the original capacity approved.

loan inspection report

The Loan Inspection Report is a service offered by the Health District for parties interested in having their on-site sewage system evaluated prior to any sale.

certification setup form

Operation & Maintenance contractors utilize the Certification Setup form to note how they "set up" the on-site sewage system. The information then becomes part of the permanent record.
