management The HIV/AIDS Case Management
Program helps clients: live as independently as possible, maintain
and improve dignity and health wherever possible, gain access to
resources and services that meet each client’s level of need,
and reduce behaviors that place them or the public at increased
risk for HIV and secondary infections. Clients drive the level of
contact and involvement and are treated with respect and confidentiality.
We make every effort to be a resource that addresses clients’
needs from the time they test HIV+ or they enroll in case management.
We work to assure that every HIV+ individual who wants or needs
services gets support.
Case management can help with
accessing:
- Medical support, food
stamps and cash support for eligible clients.
- Social security for eligible clients.
- Appropriate medical, mental health, and dental
care as needed.
- Applications for low-income housing.
- Assistance for emergency support for housing
and utility costs.
- Food and resources for clothing and furniture.
- Information and assistance for transportation.
- Assistance to get medical and dental insurance
in place.
- Referrals for mental health issues and counseling
as appropriate.
- ADATSA evaluations and treatment facilities
for in-patient or outpatient substance use treatment.
- Referrals for free or low-cost legal services
for non-criminal issues.
- Education and training in HIV issues and in
vocational
training.
- Spiritual, cultural, and social supports based
on needs expressed by the client.
- Help with safety plans related to domestic
violence or any other potentially threatening or dangerous situation.
- General support for times when clients simply
need to talk to someone.
